well, i made a bump map extrude of that... its top down view directly from above, no prespecive, looking good, light is from the right side.

this is real 3d extrusion (still a bit exagerated, not real like height)... the rivers are a bit deep, but there is no simple way around it, it would need a manual retouche and my time is too prescious for that :)
feel free to tweak the brightness/contrast to get better result, i did not do it on purspose. Its also maybe good idea to mix it with the source image to get more subtle and colorful look.
