This is the earliest I could find but it isn't the first. From 2000. I tried to model from a real bottle that is square on the bottom and round on the top.

Should have. I looked for a tool to take several shapes and wrap a minimal surface or other envelope around them. I'd seen that for nurbs somewhere. Didn't find it. Would have used it on a cube partly in an egg. So I just Boolean unioned them. The real bottle's transition is smooth.
